Blue Badge change of circumstances form

If your situation changes and if affects your Blue Badge, you need to tell us.

You can use our online form to notify us if:

  • you've changed address
  • you've changed your name (for example by marriage, civil partnership, deed poll)
  • your condition has changed, and you no longer need a Blue Badge
If you continue to display a badge when you no longer need it you may be fined up to £1,000.

If your change of circumstances requires a new badge to be printed, you'll need to:

  • pay a £10 fee
  • return your old badge by post

When we've processed your form, we'll contact you to explain how to make a payment or provide any supporting evidence.

You can post an old badge to: Blue Badge Team, Suffolk County Council, PO Box 258, Stowmarket, Suffolk, IP14 9BU.

Please use a different form if your Blue Badge has been lost, damaged or stolen, or if the badge holder has died.
