Dynamic Support Register

The register is a list of people with Learning Disabilities or Autism or both who are at risk of being admitted to hospital. 

The register makes it easier for the people who support you to respond quickly if your care isn't meeting your needs and to work together to make sure you remain safe and well. It helps services work with you to decide what support you may need. 

The register is about helping you to get the right support as fast as possible if you need it. 

This is because services will know what extra help and support you might need and can plan your support around this. You will be involved in planning your care and support.
