Home First is an in-house care service who work on behalf of Suffolk County Council.

For all of the services below from Home First you will need to be referred by a health care professional, like the hospital discharge team or your care worker.

If you're not sure if can get help with your care, our assessment tool will be able to give you an idea. It can also give you information about support in your local area; provide information about paying for care and support, and pass your details to Customer First (social services) if needed. It can be filled in by you, on behalf of someone you care for, or a professional.

Home First works with people who have been referred to them, to create a plan that will help them learn skills to be more confident living in their home.

This help can last for a maximum of six weeks however; most people achieve their goals in the first four weeks.

The plan will show people new ways to do everyday tasks, like cooking and bathing. Home First will also work with that person to help them find new ways to do more things themselves.

This support is free and aims to;

  • Stop hospital trips that are not needed
  • Help people after a hospital stay in their home
  • Stop people from moving into a care home before its needed

Through Suffolk County Council's Support to Live at Home (PDF, 139KB) initiative, Home First deliver home care services to people within the Mid Suffolk (Rural East area).

 

Home First provides county-wide care support when needed on behalf of Suffolk County Council. They are able to give temporary care to people who have been assessed by a care worker, until a permanent local care agency has been found to take over.
