If you want a job, you must ...
Get an

required)
The form must be completed and signed by your employer, parent
and headteacher.
Then send the completed form to the Child Employment
Officer.
You must also have a Health Declaration Form signed by your
parent/carer.
You cannot start work until your work permit has been returned
to you.
Always carry your work permit with you whenever you work.
If you change jobs, you must re-apply for a new work permit.
Any permit may be withdrawn or amended at any time if, in the
opinion of the Director of Education, either by reason of late
arrival or absence from school or any other reason that may affect
your education.
