Self Assessment for Community Equipment
Suffolk County Council has joined forces with the Hampshire
Coalition for Disabled People to provide a new online self
assessment service for community equipment.
Equipment Direct
is a new website that enables you to make decisions about equipment
that will help you to live independently. Features of the site
include:
- self assessment questionnaire-based upon the
answers you give to the questions, the site will produce a list of
equipment that could help you. It also provides details of where
you can purchase this equipment. Further information that may help
you choose a piece of equipment is given in the
Frequently Asked Questions
below
- self referral form – once you have completed
the self assessment questionnaire you may decide you would like to
speak to someone about your needs. By completing the self referral
form, attaching your answers and sending this to us, we can arrange
for someone to call you back to discuss your circumstances in more
detail.

speak to a Customer Service Advisor.
Anyone aged 18 or above who believes that they may have social care
needs which may fall within the definition of community care
services can ask for an assessment. Local Authorities with Social
Services responsibilities have a duty to carry out a community care
assessment when requested
