1. We promise we will only share information about you with
people who need it in order to look after you properly
2. We promise you will have a named Social Care Worker, who you
know how to contact and who will keep in touch with you.
This means:
You will know your worker’s name and have their office address,
office and mobile phone numbers and email so you can contact them
when you need to.
When you leave a message for your worker they will return your
call as soon as they can – in two working days or less. If they are
not at work another worker will contact you.
Your worker will keep in touch with you regularly to check you
are OK – at least every six weeks depending on your needs.
If your worker is going to be late or needs to cancel an
appointment with you they will let you know (this will not happen
regularly).
