Thinking about your future?
The process of leaving care and becoming independent can be
worrying and difficult. There are lots of things to think
about - things like jobs and training, money, relationships and
health. These things worry us all. The law says that if
you have been looked after, for example, in a children's home or a
foster home, the Council must provide you with help. Suffolk
have made special arrangements for young people who leave care
after their 16th birthday help and support is available via the
three specialist leaving care teams based in Ipswich, Bury and
Lowestoft. |

How will I get help? |
| Every care leaver aged between 16-21 years (and up
to 24 years if in higher education) is entitled to have a young
person's adviser. |

These advisers will be able to support you through the many
decisions you will need to make before you leave care and continue
to support you after you have left care.
Your young person's adviser, together with your social worker, will
help you prepare your own individual Pathway Plan which will
include details of what you would like to do, your education and
training needs, where you would like to work and live and plans for
your future. It will also look at what financial help you
will need to achieve your ambitions, who is around to support you
and other things that are important to you.

Where else can I get help from?
The Moving On Project might be able to help if you are not too
sure about going on with education training or work. They
offer a listening ear when your confidence needs a boost and have
good contacts with our agencies and organisations who might be able
to help you achieve what you want.

Use the guide, ask for help when you need it and remember that
your young person's adviser is trained and skilled to help
